Medications and Medical Conditions
On the first day of school, a pink sheet will be coming home with your child asking you to update your child's records. Please list all contact numbers including names and numbers of those we can call if we are unable to contact you. Also on this sheet, please include any of your child's medical conditions and medications they are taking. It is extremely important that I know of any medical conditions and medications. If this information should change during the school year, PLEASE LET ME KNOW.
If your child needs to take medication during the school day, please complete the medication consent form (available in my office) and provide the medication in the original container. Please do not send the medication with your child to school! No medication is allowed on campus with the exception of epi-pens and inhalers. If your child needs to keep an epi-pen and/or inhaler with them, there is a separate form to be completed.
The dosage you write on the medication form must match the dosage described on the medication bottle. This is true for both prescription and over-the-counter medications. If your child needs more than the dosage described on the bottle, please provide a note from your child's doctor's office.
